What to ask in a client satisfaction survey

A client satisfaction survey asks clients how well you’re serving them, so you can fix problems before they become reasons to leave. For service businesses, short and regular beats long and annual: five to eight questions, sent at milestones or every quarter.

How to fill it in

  1. Decide when you’ll send it: at milestones, quarterly or at project end.
  2. Keep it under ten questions; delete what you won’t act on.
  3. Send it from the account lead with a short personal note.
  4. Review every response within a week.
  5. Follow up personally on low scores and open comments.
  6. Track scores over time, and tell clients what you changed.

Use it in Word, Google Docs or PDF

Microsoft Word

  1. Download the Word (.docx) file and open it. If Word opens it in Protected View, select Enable Editing.
  2. Use Find (Ctrl+F or Cmd+F) and search for “[” to jump between the placeholders you need to replace.
  3. Save a clean copy as a Word template (File → Save As → Word Template) so every new document starts from it.

Google Docs

  1. Upload the .docx file to Google Drive.
  2. Right-click it and choose Open with → Google Docs, then File → Save as Google Docs to keep an editable copy.
  3. Use File → Make a copy for each new client so your master stays clean.

PDF

Fill it in Word or Google Docs, then export it: File → Save As → PDF in Word, or File → Download → PDF document in Google Docs.

Sample answers from a fictional client.

Sample answers

  • Overall satisfaction? 4 out of 5
  • Recommend? 8 out of 10
  • Communication? 3 out of 5: weekly updates sometimes arrive late.
  • Do differently? Tell us earlier when something will slip.

Tips for better answers

  • Ask the people who experience the work, not just the person who signs the invoice.
  • Always ask permission before quoting feedback publicly.
  • A low score with a comment is a gift: respond within days.

MAKE IT WORK FOR YOU

Useful structure.
Your way of working.

01

Ask only what you’ll use.

Every extra question lowers completion. Keep the ones that change what you’ll do, and move the rest to a later conversation.

02

Explain why you’re asking.

A one-line reason next to a question gets better answers than an unexplained field.

03

Act on the answers.

Review responses before your next conversation and turn them into requests with owners and dates.

GOOD QUESTIONS

Before you download.

Talk to us
How often should I survey clients?

Quarterly for ongoing relationships, and at the end of each project. More often than that, response rates fall.

How many questions should a client satisfaction survey have?

Five to ten. Short surveys get more responses, and you’ll act on more of what you learn.

What is a good recommendation score?

It depends on your industry and how you measure it. Track your own trend over time rather than chasing a benchmark.

Can I edit this template in Word or Google Docs?

Yes. Download the Word (.docx) file and open it in Microsoft Word, or upload it to Google Drive and choose Open with → Google Docs. There’s also a Markdown version for any text editor. No account or email address is needed.

How do I save it as a PDF?

Fill it in, then export it: File → Save As → PDF in Word, or File → Download → PDF document in Google Docs. On this page you can also print just the template or save it as a PDF from your browser’s print dialog.
