When to ask for client feedback

A client feedback form at the end of a project captures what worked while it’s fresh, surfaces problems you can still fix, and gives you testimonials and referrals. Send it within a week of handover, with a personal note.

For a regular pulse on ongoing work, use the client satisfaction survey instead.

Use it in Word, Google Docs or PDF

Microsoft Word

  1. Download the Word (.docx) file and open it. If Word opens it in Protected View, select Enable Editing.
  2. Use Find (Ctrl+F or Cmd+F) and search for “[” to jump between the placeholders you need to replace.
  3. Save a clean copy as a Word template (File → Save As → Word Template) so every new document starts from it.

Google Docs

  1. Upload the .docx file to Google Drive.
  2. Right-click it and choose Open with → Google Docs, then File → Save as Google Docs to keep an editable copy.
  3. Use File → Make a copy for each new client so your master stays clean.

PDF

Fill it in Word or Google Docs, then export it: File → Save As → PDF in Word, or File → Download → PDF document in Google Docs.

Sample answers from a fictional client.

Sample answers

  • Achieve what you hoped? Yes — online bookings are up and phone calls are down.
  • Done differently? We’d have liked the content deadline explained earlier.
  • Testimonial? Yes.

Tips for better answers

  • Ask about outcomes, not just service.
  • Make testimonial permission an explicit yes/no.
  • Close the loop: tell the client what you’ll change.

MAKE IT WORK FOR YOU

Useful structure.
Your way of working.

01

Ask only what you’ll use.

Every extra question lowers completion. Keep the ones that change what you’ll do, and move the rest to a later conversation.

02

Explain why you’re asking.

A one-line reason next to a question gets better answers than an unexplained field.

03

Act on the answers.

Review responses before your next conversation and turn them into requests with owners and dates.

GOOD QUESTIONS

Before you download.

Talk to us
What questions should a client feedback form ask?

Whether the project met its goals, what went well, what to improve, how communication felt, the outcome so far, and permission to use comments as a testimonial.

Should client feedback be anonymous?

For end-of-project feedback, usually not: you want to follow up. For broad satisfaction surveys across many clients, anonymity can produce franker answers.

How do I get more clients to respond?

Keep it short, send it personally from someone they worked with, and explain how you’ll use the answers.

Can I edit this template in Word or Google Docs?

Yes. Download the Word (.docx) file and open it in Microsoft Word, or upload it to Google Drive and choose Open with → Google Docs. There’s also a Markdown version for any text editor. No account or email address is needed.

How do I save it as a PDF?

Fill it in, then export it: File → Save As → PDF in Word, or File → Download → PDF document in Google Docs. On this page you can also print just the template or save it as a PDF from your browser’s print dialog.
