What is a project intake form?

A project intake form captures every new request in the same shape, so you can decide quickly whether to take it on, how to prioritise it and what it needs. It’s different from a client intake form, which qualifies a new client rather than a new piece of work.

How to fill it in

  1. Make the form the single way new requests arrive.
  2. Require the goal and deadline; make the rest optional.
  3. Triage new requests on a set day each week.
  4. Reply with a decision, estimate or questions.
  5. Turn accepted requests into scoped work with owners and dates.
  6. Close the loop on declined requests with a reason.

Use it in Word, Google Docs or PDF

Microsoft Word

  1. Download the Word (.docx) file and open it. If Word opens it in Protected View, select Enable Editing.
  2. Use Find (Ctrl+F or Cmd+F) and search for “[” to jump between the placeholders you need to replace.
  3. Save a clean copy as a Word template (File → Save As → Word Template) so every new document starts from it.

Google Docs

  1. Upload the .docx file to Google Drive.
  2. Right-click it and choose Open with → Google Docs, then File → Save as Google Docs to keep an editable copy.
  3. Use File → Make a copy for each new client so your master stays clean.

PDF

Fill it in Word or Google Docs, then export it: File → Save As → PDF in Word, or File → Download → PDF document in Google Docs.

Example: Tidewater Marketing request from Larkspur Interiors

Sample answers from a fictional client.

Sample answers

  • What do you need? A landing page for the spring campaign.
  • Goal? Capture consultation enquiries from paid ads.
  • When? Live by 1 March, when ads launch.
  • Approver? Head of marketing.

Tips for better answers

  • Ask why the deadline matters: many deadlines move when questioned.
  • Capture the approver up front.
  • Use the same form for every client so requests are comparable.

MAKE IT WORK FOR YOU

Useful structure.
Your way of working.

01

Ask only what you’ll use.

Every extra question lowers completion. Keep the ones that change what you’ll do, and move the rest to a later conversation.

02

Explain why you’re asking.

A one-line reason next to a question gets better answers than an unexplained field.

03

Act on the answers.

Review responses before your next conversation and turn them into requests with owners and dates.

GOOD QUESTIONS

Before you download.

Talk to us
What should a project intake form include?

The requester, approver, what’s needed, the goal, deliverables, deadline and reason for it, budget, priority and dependencies.

Who reviews project intake forms?

Usually an account or project manager who triages requests on a regular schedule and agrees priorities with the client.

Project intake vs client intake?

Client intake qualifies a new client relationship. Project intake captures a specific new piece of work, often from an existing client.
