Using a tax intake form

A tax intake form collects what you need to prepare a return before the first meeting: who the client is, what changed this year, their income sources and which documents to send. Tax rules and required information vary by country and state, so adapt the questions to your jurisdiction.

Never collect identity numbers or financial documents by email. Use a secure upload, such as a client document request portal, and follow your identity-check obligations.

Use it in Word, Google Docs or PDF

Microsoft Word

  1. Download the Word (.docx) file and open it. If Word opens it in Protected View, select Enable Editing.
  2. Use Find (Ctrl+F or Cmd+F) and search for “[” to jump between the placeholders you need to replace.
  3. Save a clean copy as a Word template (File → Save As → Word Template) so every new document starts from it.

Google Docs

  1. Upload the .docx file to Google Drive.
  2. Right-click it and choose Open with → Google Docs, then File → Save as Google Docs to keep an editable copy.
  3. Use File → Make a copy for each new client so your master stays clean.

PDF

Fill it in Word or Google Docs, then export it: File → Save As → PDF in Word, or File → Download → PDF document in Google Docs.

  • Income? Salary to March; freelance design income from April.
  • Deductions? Home office, software subscriptions, a new laptop.

Tips for better answers

  • Ask what changed this year first; it surfaces the complicated items.
  • Give a document deadline well ahead of the filing deadline.
  • Use a checklist so clients can see what’s still outstanding.

MAKE IT WORK FOR YOU

Useful structure.
Your way of working.

01

Ask only what you’ll use.

Every extra question lowers completion. Keep the ones that change what you’ll do, and move the rest to a later conversation.

02

Explain why you’re asking.

A one-line reason next to a question gets better answers than an unexplained field.

03

Act on the answers.

Review responses before your next conversation and turn them into requests with owners and dates.
