Get what you need.

Build intake forms and document requests with file uploads, signature fields and payment fields. Share them by link or assign them in the client portal.

Forms are on every plan.

What you can build

Client forms in Angelwood cover intake questionnaires, document requests and simple orders. Fields include text, long text, email, phone, dates, dropdowns, checkboxes, file upload, signature and payment, so one form can collect answers, documents, a signature and a payment.

Describe the form you need and Generate with AI builds a first draft for you to edit. Form settings cover public access, a maximum number of submissions, an expiry date, notification emails and CAPTCHA.

Reviewing what comes back

Submissions show completion and status. Open one to check the answers and files, then select Mark as Reviewed. Export submissions as CSV or JSON. For secure document collection workflows, see the client document request portal.
